National Youth in Transition Database
The National Youth in Transition Database or NYTD was created by the federal government. It is a tool for states to use to collect information on services provided to current and former foster youth. It also helps track the outcomes of certain foster youth. Having this information will help both the state and federal programs to improve services and support for foster youth. Youth voice matters!
Youth who fall into a survey group will be notified to complete a Survey (40 KB PDF) that asks questions about how they are doing. It asks about school, employment, supportive services, etc. The Survey takes about 10 minutes to complete. Youth will receive an email and a letter that will tell them how to complete the Survey.
How can youth help us make sure they receive their survey?
- Provide the DCS case manager with the best email address available
- Contact us after you leave foster care to let us know when your contact information changes (email and postal address, phone number, etc.)
