Roll Up Your Sleeve Flu Shot Incentive Program
Protect yourself and your family from the flu this winter.
CMDP is offering a $10 eGift card to AHCCCS enrolled children in DCS out-of-home care who receive a flu shot after September 1, 2020.
Children can get flu shots at doctor’s offices, immunization clinics and community health centers around the state. Many pharmacies are able to administer flu shots to children between the ages of three and 18 years. CMDP covers flu shots for children in DCS care at no cost to you.
Follow these easy steps and qualify to receive a $10 eGift card:
- Make an appointment for the child to receive a flu shot with any AHCCCS registered provider, healthcare clinic or pharmacy. Please call CMDP Member Services at 1-800-201-1795 (TTY/TDD 711) if you need help finding a location offering flu shots near you.
- After the child receives their flu shot, fill out the online web form below or click here.
- If you need help completing the web form, contact CMDP Member Services at 1-800-201-1795 (TTY/TDD 711) or email [email protected]. Be sure to have the child’s name, CMDP ID or AHCCCS ID number, date of birth, provider name and the date the flu shot was received.
When CMDP verifies the flu shot from the provider, an eGift card is sent via SMS/Text or Email depending on your selection below. Please note that it may take up to six months for CMDP to receive the claim from the provider.
