Effective Monday, October 1, 2018, CMDP will be using OptumRx as the new Pharmacy Benefits Manager (PBM) to provide prescription coverage for CMDP Members.

Prior Authorization
Effective 10/1/2017 – Any expedited prior authorization being requested outside of normal business hours (Monday-Friday, 8a-5p and on any state observed holiday) needs to be faxed to 602.351.8529 as well as sent electronically to the following email address: [email protected] to ensure timely processing.
