A DCS CHP Resource Coordination team member is ready to assist you to discuss available tools and resources to help guide you toward the child’s/youth’s wellness goals. Our team members can help with:

• Reviewing health care benefits including well-child (EPSDT), dental, vision and behavioral health services
• Using the Member Handbook (English) or Manual para Miembros (Spanish) and Member ID Card
• Selecting healthcare providers and pharmacies using the Find a Provider or Pharmacy Tool
• Scheduling health appointments needed within the first 30 days of entry into out of home care
• Scheduling non-emergency medical transportation
• Expect an Integrated Rapid Response Evaluation for the child in your care to receive a behavioral health assessment and physical screening within 72 hours of removal. If you haven’t received a call to arrange an appointment within 24 to 96 hours of the child arriving in your home, you can call the Arizona Foster Care Hotline at 602-633-0763.
