Arizona Department of Child Safety
Phone: Child Abuse Hotline 1-888-767-2445
Phoenix, AZ (Friday, June 05, 2015) – Investigators from the DCS Office of Child Welfare Investigations (OCWI) are working with the Mesa Police Department in that department’s criminal investigation into the death of a 1 year-old child earlier today.
Upon receiving the report of the child’s death and the subsequent arrest of 27 year-old Andrew Isaac, DCS was able to confirm that Isaac is named as a perpetrator in a December, 2014 report to the DCS Hotline alleging abuse of this child.
It appears as though the assigned DCS specialist made only one attempt to contact this family and was unsuccessful.
Director Greg McKay stated, “When allegations of abuse or neglect are made, the Department of Child Safety has a responsibility to confirm that children are safe. We are fully committed to conducting an in depth review of cases that result in tragic outcomes and what, if any, part we played. This is yet another dreadful reminder of the violence people are capable of and the devastating consequence on our most vulnerable citizens."
