How do I locate health providers for the child in my care?

You can select a health care provider from the Mercy Care DCS CHP Provider Directory. The directory includes languages that the provider speaks and services they provide to those with physical disabilities. You can search the list of providers by location, specialty or name. The directory is available on the Mercy Care DCS CHP website at www.MercyCareAZ.org (link is external), or call Member Services at 602-212-4983 or 1-833-711-0776 (TTY/TDD 711), to have a copy of the directory mailed to you at no cost.

What is the Division of Developmental Disabilities (DDD)?

The Division of Developmental Disabilities (DDD) is a service system administered through the Arizona Department of Economic Security (DES) that supports people who develop severe and/or chronic disabilities that may limit a person's ability to do the tasks related to daily living. If a child is enrolled in DDD, a support coordinator will be assigned to help determine a service plan to meet the needs of the child. What are the different types of services covered by Arizona Long Term Care (ALTCS)?

  • Institutional care in a nursing facility;
  • Home and community based services, combining out-patient and in-home care;
  • Medical services, such as doctor’s office visits and prescriptions (prescription coverage is limited for people with Medicare);
  • Behavioral health services;
  • Preventive and well care for children; and
  • Hospice services.

What is the Mercy Care DCS Comprehensive Health Plan?

The Mercy Care Department of Child Safety Comprehensive Health Plan (Mercy Care DCS CHP) is the single, statewide health plan, responsible for the physical, dental, and behavioral health services for all Arizona children in foster care.

What is a Prior Authorization (PA)?

A Prior Authorization (PA) is when a provider calls Mercy Care DCS CHP for approval to provide special services. It’s up to the child’s health care provider to get a PA from Mercy Care DCS CHP. Mercy Care DCS CHP reviews the service request from the member’s provider. The provider will tell you if the child’s service is approved.

What documentation and notification is required following a crisis?

Licensed foster parents should notify DCS and their licensing agency within 24 hours if their child has an illness, injury, change of medication, or medication error that results in seeking medical attention.

Licensed foster parents should notify DCS and their licensing agency immediately if their child dies, has a serious illness or injury requiring hospitalization, or receives urgent care or emergency room treatment and whenever emergency services (911) is contacted. Within 24 hours of any of the above noted incidents, licensed foster parents shall complete an Unusual Incident Report form and submit it to their licensing agency.

Unlicensed kinship foster parents are to notify DCS within 2 hours of the following the death of a child or hospitalization (medical or psychiatric). Unlicensed kinship foster parents are to notify DCS within 48 hours if the child becomes seriously ill.

What should I do if I have a grievance regarding any services that my child is receiving?

If there are concerns regarding the services the child is receiving you can contact the specific health plan for the child or the Division of Developmental Disabilities Customer Service Center 1-844-770-9500 (toll free) or DDDCustomerServiceCenter@azdes.gov

What pharmacy should I go to?

All prescriptions must be filled at a pharmacy in Mercy Care DCS CHP’s network. If you need pharmacy services after hours, on weekends or holidays, many pharmacies are open 24 hours, 7 days a week. You can find a list of pharmacies in the Mercy Care DCS CHP Provider Directory. Visit www.MercyCareAZ.org (link is external) and select “For Members” under Mercy Care DCS CHP. You’ll locate “Find a Pharmacy” in the upper right corner of the screen.

Which immunizations (shots) are recommended at which age?

Immunizations (shots) can keep children and youth from getting sick in the future. Talk with the child’s Primary Care Provider (PCP) about the immunizations that are needed and when they are needed. You should use an immunization schedule and have the schedule updated when you visit the child’s doctor. For more information on vaccines and to review recommended immunization schedules, visit the Centers for Disease Control and Prevention at www.cdc.gov/vaccines/schedules/ (link is external) or call Mercy Care DCS CHP Member Services at 602-212-4983 or 1-833-711-0776 (TTY/TDD 711).

Are medications covered by Mercy Care DCS CHP?

Prescription medications and some over-the-counter medications are covered by Mercy Care DCS CHP. If the child needs medicine, the child’s health care provider will choose one from Mercy Care DCS CHP’s list of covered medications (called a formulary) and write a prescription. If you want a copy of the list, call Mercy Care DCS CHP Member Services or go to the website at www.MercyCareAZ.org (link is external). If you are at a pharmacy and the pharmacy tells you that Mercy Care DCS CHP will not pay for the medication, call Mercy Care DCS CHP Member Services right away. Do not pay out of your own pocket for the medicine. If you have questions or problems outside Mercy Care business hours, you can call the Mercy Care 24-hour nurse line at 602-212-4983 or 1-833-711-0776 (TTY/TDD 711).

How do I get the child's immunization records?

The Arizona Department of Health Services (DHS) maintains an electronic registry of immunizations, and Arizona providers are mandated by law to report all immunizations of children ages birth through 18 in Arizona.

To get a copy of a child's immunization records, fill out the ADHS Immunization Record Request Form (link is external), and fax (602-364-3285) or email ASIISHelpdesk@azdhs.gov the form along with a copy of the Notice to Provider, a copy of your driver's license, and instructions for where the records should be faxed, emailed, or mailed back to you.

You can also pick up a copy of a child's immunization records in person at the Arizona Department of Health Services, Immunization Program Office at 150 N. 18th Avenue, Suite 120 Phoenix, AZ 85007 by bringing the notice to provider and a valid ID.

Your DCS case manager or DDD support coordinator can also assist in obtaining the child's immunization records.

What should I do if I am having trouble filling a prescription for the child?

If you have any questions or trouble filling a prescription while you are at the pharmacy, you can call Mercy Care DCS CHP Member Services for assistance. If you have questions or problems outside Mercy Care business hours, you can call the Mercy Care 24-hour nurse line at 602-212-4983 or 1-833-711-0776 (TTY/TDD 711).

How can I get a Mercy Care DCS CHP insurance card for the child?

Member Mercy Care DCS CHP Member ID cards are distributed through a variety of means including the DCS Specialist as well as directly to caregivers when possible. If you did not receive a member ID card or if you need a replacement card, contact Mercy Care DCS CHP member services.

How often does the child in my care need to go to the dentist?

It is important to start dental care at an early age and continue regularly. Children and youth who are at least 1 year of age, or with the eruption of the first tooth, should receive a dental examination within 30 days of removal and placement into foster care. Regular dental checkups should occur every 6 months following the first visit.
