How do I organize and store the child's information and records?

It is a good idea to keep the child's records in a notebook or binder that you can bring to Child and Family Team (CFT) meetings and other appointments. This will help you organize and store all of the child’s records, information, and documents. If the child moves to a new foster home, send this notebook/binder to the child's DCS Specialist.  Also, remember that the child's case record is confidential. Be sure to keep the child's records stored in a secure location in your home that only caregiver (you) can access.

What dental appointments does the child need?

Caregivers must schedule a dental examination for each child age 1 year or older as soon as possible but no later than within 30 days of placement. Regular dental checkups should occur every 6 months following the initial dental examination.

What wellness appointments does the child need?

Caregivers must schedule a well-child visit, also known as an Early Periodic Screening, Diagnostic and Treatment (EPSDT) visit, a dental visit and behavioral health evaluation for each child within the first 30 days of the child’s removal and placement into care.  

My child got hurt; who should I contact?

Licensed foster parents are to notify DCS and their licensing agency within 24 hours if their child has an illness, injury, change of medication, or medication error that results in seeking medical attention.

Licensed foster parents are to notify DCS and their licensing agency immediately if their child dies, has a serious illness or injury requiring hospitalization, or receives urgent care or emergency room treatment and whenever emergency services (911) is contacted.

Within 24 hours of any of the above noted incidents, licensed foster parents shall complete an Unusual Incident Report form and submit it to their licensing agency.

Unlicensed kinship foster parents are to notify DCS within 2 hours of the following: death of a child or hospitalization (medical or psychiatric.

Unlicensed kinship foster parents are to notify DCS within 48 hours if the child becomes seriously ill.

What health insurance does the child in my care have?

Mercy Care Department of Child Safety Comprehensive Health Plan (Mercy Care DCS CHP) is the health plan for Arizona’s children in foster care. Mercy Care DCS CHP is a program administered by DCS. Children in foster care are automatically enrolled in Mercy Care DCS CHP.

What is Arizona Long Term Care System (ALTCS)?

Certain disabilities qualify children for Arizona Long Term Care System (ALTCS). ALTCS is the State of Arizona's Medicaid program that funds long-term care services, for financially and medically eligible Arizona residents. Once eligibility has been determined for ALTCS services, the child will be enrolled with a Health Plan (Mercy Care, Care 1st or United Healthcare). The health plan will provide the services the child is qualified for.

Who can I contact in a crisis?

For all medical emergencies, dial 911 immediately. If you or a child or youth in your care are facing a behavioral health crisis, you can call one of the Arizona crisis lines listed below. Call anytime, 24/7, for crisis intervention, support and referrals.

  • Northern Arizona Crisis Line (serves Apache, Coconino, Gila, Mohave, Navajo and Yavapai counties): 1-877-756-4090

Which pharmacy should I go to?

You can find a list of pharmacies in the Mercy Care DCS CHP Provider Directory. Visit www.MercyCareAZ.org and select “For Members” under Mercy Care DCS CHP. You’ll locate “Find a Pharmacy” in the upper right corner of the screen.

How do I get a Mercy Care DCS CHP member identification (ID) card for the child in my care?

Member ID cards are distributed through a variety of means including the DCS Specialist as well as directly to caregivers when possible. If you did not receive a member ID card or if you need a replacement card, contact Mercy Care DCS CHP Member Services.

Do caregivers need to pay for services provided for an Arizona Long Term Care (ALTCS) eligible child?

Caregivers do not need to pay for ALTCS services. If you get a bill for a service that is covered by ALTCS, contact the agency that sent you the bill. You are not responsible to pay out of pocket costs, including AHCCCS co-payments.

How do I locate health providers for the child in my care?

You can select a health care provider from the Mercy Care DCS CHP Provider Directory. The directory includes languages that the provider speaks and services they provide to those with physical disabilities. You can search the list of providers by location, specialty or name. The directory is available on the Mercy Care DCS CHP website at www.MercyCareAZ.org, or call Member Services at 602-212-4983 or 1-833-711-0776 (TTY/TDD 711), to have a copy of the directory mailed to you at no cost.

What are the different types of services covered by Arizona Long Term Care (ALTCS)?

  • Institutional care in a nursing facility;
  • Home and community based services, combining out-patient and in-home care;
  • Medical services, such as doctor’s office visits and prescriptions (prescription coverage is limited for people with Medicare);
  • Behavioral health services;
  • Preventive and well care for children; and
  • Hospice services.

What is a Prior Authorization (PA)?

A Prior Authorization (PA) is when a provider calls Mercy Care DCS CHP for approval to provide special services. It’s up to the child’s health care provider to get a PA from Mercy Care DCS CHP. Mercy Care DCS CHP reviews the service request from the member’s provider. The provider will tell you if the child’s service is approved.

What should I do if I have a grievance regarding any services that my child is receiving?

If there are concerns regarding the services the child is receiving you can contact the specific health plan for the child or the Division of Developmental Disabilities Customer Service Center 1-844-770-9500 (toll free) or [email protected]

Which immunizations (shots) are recommended at which age?

Immunizations (shots) can keep children and youth from getting sick in the future. Talk with the child’s Primary Care Provider (PCP) about the immunizations that are needed and when they are needed. You should use an immunization schedule and have the schedule updated when you visit the child’s doctor.
