For DCS specialist contact, please call their assigned desk phone or cell and leave a voicemail. You may also email your assigned specialist. If your case manager is not available, call their supervisor and leave a voicemail. If you know the local DCS office that your case is assigned to and need the front office phone to assist in getting ahold of your case manager or supervisor, please see our Field Offices page. From there, you can contact your assigned office and speak to the secretary who can forward you to your assigned case manager’s phone.
If you do not know who your assigned DCS Specialist is, you can call the hotline to request your assigned specialist/supervisor to contact you.

Case File Records

If you are a Biological Parent, Adoptive Parent, Prospective Adoptive Parent, Legal Guardian or an Attorney, who is, or was, involved in a DCS report or investigation, and need case file records (DCS Report Summaries, case notes, court reports, case plans, etc. or other specific documents), please click and open form CSO-1036A or CSO-1036AS for Spanish. The form is required for any case file records requests. For Central Registry Checks only, please see the Child Abuse and/or Neglect Background Checks section of the table. Select the specific option on the page, and using the links displayed, determine if that item is already publicly available. Read the information about report availability. If you choose to continue with the request, fill in the required fields and select the specific option on the Request for Information form, then click the Submit button at the bottom of that page to submit your request. Out-of-State Child Protective Agencies

If you are an out-of-state child protective agency seeking Arizona DCS records, please email your request from your state government email account to [email protected].

Child Abuse and/or Neglect Background Checks (Central Registry checks)
|
To obtain background check information on whether a person(s) has a Child Abuse/Neglect history (Central Registry) in the State of Arizona, there are different procedures based on your type of request below. The results of these requests will only show if there is a Substantiated report on the DCS Central Registry. No case details are provided. For specific case details, please see the Case File Records request section of this page.
Employment:
A. Arizona Public Sector and Service Providers:
- For employment/volunteer purposes, please use Arizona’s NEW Centralized Background Check System. This system requires both the employer and prospective employee to register, and the prospective employee is responsible for selecting the prospective employer to receive the results of their background check. Fingerprint clearance status updates (DPS), Adult Protective Services (APS) and Department of Child Safety (DCS) background check results are all included. Visit https://des.az.gov/cbc to register as an employer or prospective employee and request a background check.
B. Out of State requestors:

Prospective Caregivers for Foster Care / Adoption (Adam Walsh) Checks:
- For placement of foster care/kinship/adoption purposes, please use our CSO-1058A form (for Spanish, open CSO-1058A-S). These forms may show as 2 pages. Both pages MUST be included in a single attachment.
The forms, DCS-1083A and CSO-1058A, are agency only forms and the results will be returned to the requesting agency listed on the form(s). Please make sure all agency information listed is provided in case there are questions or issues.
Individuals:
- For individual central registry background check purposes, please use our CSO-2040 form and sign before a notary.
